He said 174 gay or bisexual African-American participants in his study had admitted they did not feel comfortable within the black community, which was usually religious and family-orientated and often homophobic.

Equally, it was difficult to turn to a gay community which was often racist, while having sex with men also increased concern about sexually transmitted diseases and gender role stress.

In dealing with this "double minority status", men often developed stress-related problems with self esteem, depression and anxiety, and these in turn manifested themselves as sexual problems, Prof Zamboni said.

He said the problem could be compounded by doctors who tried to be politically correct by not talking about race or sexuality with patients, and overlooked the source of the problem.

Speaking to AAP, Prof Zamboni said: "If you ignore that and just pretend they're like someone else - you're trying to be PC (politically correct) and colourblind and you think you're being sensitive.

"You're ignoring a unique source of stress that may be causing the low sexual desire, the erection problem, premature ejaculation.

"(Doctors) should ask about it in the first session ...
